Job Summary:

Responsibilities:

  • Implement and manage patient care plans in accordance with physician orders and medical guidelines.
  • Conduct thorough patient assessments, identifying and addressing any changes in condition or needs.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document all patient interactions, care provided, and outcomes in a timely and accurate manner.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Stay up-to-date with the latest medical research, technologies, and best practices in patient care.

Requirements:

  • Current, unencumbered RN license in the state of practice.
  • Current CPR certification.
  • One year of experience in a clinical care setting, preferably in pediatrics or home health.
  • Excellent communication, problem-solving, and time management skills.
  • Ability to work independently and as part of a team.
  • Strong attention to detail and ability to maintain accurate records.

Preferred Qualifications:

  • Home health experience.
  • Experience working with medically fragile patients.
