Primary Care Provider for Elderly Patients

We are seeking a dedicated and compassionate Primary Care Provider to join our team at Visiting Docs. As a Nurse Practitioner or Physician Assistant, you will be responsible for providing high-quality, in-person primary care services to patients in their residences or assisted living facilities.
Key Responsibilities:
Provide general primary care services to patients, including diagnosing, treating, and managing acute and chronic conditions.
Prescribe and refill medications as necessary.
Communicate effectively with patients, families, and other healthcare professionals.
Order and interpret diagnostic tests, such as imaging and lab work.
Document patient histories and treatments in our electronic medical record system.
Qualifications:
Active, unrestricted New York state license as a Nurse Practitioner or Physician Assistant.
Minimum three years of experience preferred.
PECOS enrollment and DEA certification required.
BCLS certification.
Knowledge of eClinicalworks preferred.
