Clinical Leadership Opportunity
**About the Role**
We are seeking a skilled Clinical Manager RN to lead our Home Health Services team. As a key member of our leadership team, you will be responsible for directing the overall operation of our clinical services, providing oversight and coordination of referrals, and maintaining continuity of patient care in a home care setting.
**Responsibilities**
* Direct the overall operation of the clinical services within the Home Care Home Based (HCHB) system
* Provide oversight coordinating referrals to the agency with internal and external referral sources
* Direct and follow established policies and procedures as related to the scope of practice
* Maintain continuity of patient care in a home care setting
* Proactive, collaborative team member in the home care environment
* Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
* Two (2) years recent practical experience as an RN
* One (1) year experience in Home Health required
