PruittHealth Hospice is seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a RN Case Manager, you will play a vital role in providing high-quality care to our patients and their families.
Key Responsibilities- Coordinate the interdisciplinary care team for assigned patients and implement the plan of care.
- Provide primary nursing care for hospice patients, regardless of the setting.
- Assume full responsibility for professional management of assigned hospice patients' care related to their terminal illness.
- Teach primary caregivers skills needed to care for patients in their residence.
- Work closely with attending physicians and other team members to develop appropriate pain and symptom management.
- Complete nursing assessments and admissions of patients.
- Maintain clinical records to meet federal, state, and agency regulations and guidelines.
- Supervise hospice aides and certified nurse aides.
- Assume on-call responsibilities as assigned.
- Participate in Performance Improvement Program.
- Participate in and present inservice programs, team conferences, and staff meetings.
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
