TheraCare of New York is seeking a motivated Physical Therapist to deliver exceptional care and enthusiasm in working with our adult population in our Skilled Nursing Community in La Porte, Indiana.
About the Role:The Physical Therapist will evaluate and treat patients, in accordance with the orders from the physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination, body alignment, to relieve pain, improve movement and function.
Responsibilities:- Evaluate and treat patients in accordance with physician orders
- Develop and implement individualized treatment plans
- Provide therapy services in locations including Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities, or Home Health Care
- Collaborate with healthcare teams to ensure comprehensive patient care
- Document patient progress and maintain accurate records
- Bachelor of Science or advanced degree in Physical Therapy
- Current state license/certification
- Current registration with the American Physical Therapy Certification Board
- Prefer minimum one (1) year experience in rehabilitation, but new grads are welcome to apply
- Excellent oral, written communication, and interpersonal skills
- Willingness to provide therapy services in various locations
