We are seeking a skilled Registered Nurse to join our team at Residential Home Health and Hospice. As a Hospice Care Team Member, you will play a vital role in providing exceptional care to our patients and their families.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Provide education and support to patients and families on end-of-life care and disease progression
  • Communicate effectively with healthcare providers and facilities to ensure safe and successful discharges
Requirements:
  • Associate degree from an accredited School of Nursing
  • Minimum of two years of experience working as a Registered Nurse, including acute care or oncology experience
  • Experience with technologies such as Microsoft Office Suite, EPIC, and HCHB
  • Ability to transfer patients safely and efficiently
