Job Title: Family Advocate Support Worker

Pathway Family Services LLC is seeking a dedicated and compassionate Family Advocate Support Worker to join our team. As a key member of our Independent Living Program, you will play a vital role in promoting the well-being of youth residing in our Teens Reaching Adulthood and Independent Living (TRAIL) program.

Key Responsibilities:
  • Provide direct supervision and support to youth in onsite apartments, promoting a safe and healthy living environment.
  • Teach independent living skills, including shopping, housekeeping, budgeting, job skills, personal hygiene, health, and peer interactions.
  • Collaborate with staff from other agencies to ensure compliance with individualized written plans and provide educational and situational counseling to adolescents.
  • Monitor youth participation in community resources, promoting employment, education, mentorship, and volunteerism.
  • Complete documentation and reports, including descriptive, analytical, and evaluative content.
Requirements:
  • High School Diploma/G.E.D. required; Bachelor's degree in Human Services preferred.
  • Valid driver's license and meet agency underwriting standards.
  • Ability to work flexible hours, including evenings and weekends, and provide on-call support as necessary.
