We are seeking a highly skilled and compassionate Intake Nurse to join our team at Johnstown Heights Behavioral Health, LLC. As an Intake Nurse, you will play a critical role in assessing patients and determining their level of care.
Purpose StatementThe Intake Nurse will perform assessments at or outside the facility, depending on the needs of the patient or hospital. This will involve screening and assessing admissions to evaluate clinical and medical appropriateness and communicating this information to a physician who makes the admissions and level of care decision.
Roles and Responsibilities- Facilitate admissions and assessment processes for incoming patients.
- Provide initial evaluation and initiate the integrated assessment, documenting patient symptoms, functioning, and needs.
- Collaborate with other facility medical and psychiatric staff to ensure appropriate recommendations and admissions.
- Communicate projected admissions to designated internal representatives in a timely manner.
- Ensure all medical admission documentation is gathered from external sources prior to patient admission and secure initial pre-authorization for treatment and admission.
- Review patient charts to determine medical necessity and staff each referral and assessment with appropriate physician to obtain orders, recommendations, and DSM-V diagnosis, when appropriate.
- Communicate treatment recommendations to and coordinate with other treatment teams.
- May administer patients' medications and follow all physician's orders in consultation with psychiatrist and in accordance with established hospital and licensing agency protocols.
- May maintain accountability of administration, recording, control, and security of medications, including narcotics. Assure completion of Adverse Drug Reaction forms as necessary.
- Advocate for the lowest level of care consistent with the patient's needs. Coordinate admission and transfer between levels of care within the facility.
- Assure thorough, accurate documentation is completed for all patients on each shift and facilitate the response to requests for services, including the initial response, the assessment, and referral process, the designation of appropriate level of care, initiation/admission into services, and appropriate follow-up activity.
Graduation from an accredited school of nursing is required. BSN is preferred. One or more years' experience in mental health or substance use disorder admissions is preferred. Experience in clinical interviewing, patient assessment, referral, treatment planning, communicating with external review organizations or comparable entities is required. Experience in the managed care pre-certification process, level of care assessments, and utilization is preferred. Outstanding interpersonal and interviewing and assessment skills are required. Skill in telephone etiquette and paging procedures is required.
Licenses/Designations/CertificationsA valid current RN license from the state or from a compact state, where applicable, is required. CPR and de-escalation and restraint certification is required (training available upon hire and offered by facility). First aid may be required based on state or facility requirements.
Supervisory RequirementsThis position is an Individual Contributor.
