Occupational Therapist

BAYADA Home Health Care is seeking a skilled Occupational Therapist to join our team in Waianae, Hawaii.

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our clients in their homes. As a member of our team, you will work closely with our multidisciplinary healthcare team to develop and implement individualized treatment plans that promote independence and improve the overall well-being of our clients.

• Conduct thorough evaluations of clients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
• Develop and implement individualized intervention plans to achieve favorable outcomes.
• Evaluate home environments for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
• Continually assess and revise the occupational therapy care plan and participate with nursing in the multidisciplinary care plan.
• Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.

• Licensed as an Occupational Therapist in HI.
• 1 year of experience as an Occupational Therapist preferred.
• Ability to travel in assigned territory.
