Geriatric Social Worker Job Description

At EAC Network, we are seeking a compassionate and skilled Geriatric Social Worker to join our team in Port Washington, NY. As a Geriatric Social Worker, you will play a vital role in providing comprehensive social work services to older adults in the Town of North Hempstead.

Key Responsibilities:
  • Conduct thorough psychosocial evaluations to assess the needs of older adults and develop personalized care plans.
  • Screen and assist clients with benefit and entitlement applications to ensure they receive the support they need.
  • Provide individual and group counseling to help clients cope with challenges related to aging.
  • Make referrals to community resources to support clients in need.
  • Maintain client records, statistics, and reports in line with HIPAA guidelines and Project Independence requirements.
  • Build relationships with local organizations and educate the community on available resources through Project Independence.
  • Assist the Department of Services for the Aging (DOSA) with outreach efforts to connect more seniors with essential services.
Requirements:
  • Master's Degree in Social Work (MSW) from an accredited program; a degree in Gerontology may be considered.
  • Knowledge of Nassau County services and the specific needs of veterans and the geriatric population.
  • Ability to understand and address the needs of older residents; strong verbal and written communication skills.
  • Proficient in Microsoft Office, email, and database software, and able to work with a laptop for documentation.
  • Fluency in English required; Spanish, French/Creole, or Farsi is a plus.
  • Must have access to a car and a valid NYS Driver's License to travel within the Town of North Hempstead.
