Job Summary

The Social Worker will serve as a vital member of a multidisciplinary treatment team, responsible for coordinating the care of patients on assigned caseloads. This role requires a strong understanding of psychosocial evaluation, treatment planning, and patient advocacy.

Key Responsibilities
  • Conduct thorough psychosocial evaluations to inform treatment plans
  • Collaborate with team members to develop and implement person-centered treatment plans
  • Ensure accurate and timely documentation of patient interactions and progress
  • Provide supervision and guidance to interns as needed
  • Participate in quality assurance and outcome measures initiatives
  • Develop and maintain knowledge of community resources and services
  • Arrange for post-discharge care and referrals as necessary
  • Collect and analyze data to inform treatment decisions and report to the Clinical Director
Requirements
  • Master's degree in Social Work or Mental Health Counseling from an accredited institution
  • LMSW/LCSW/LMHC license required
  • Experience in inpatient settings preferred
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and adapt to changing situations
