  • Full-time, permanent position with the flexibility to work with patients in their homes, including a daytime schedule and on-call rotation.
  • Attractive compensation package featuring health, dental, and vision benefits, life insurance, dependent care reimbursement, short-term and long-term disability, 401k matching with immediate vesting, paid time off, floating holidays, transportation reimbursement, and educational support.
  • Competitive salary range of $110,000 - $137,000, commensurate with experience.

  • Deliver high-quality care while adhering to company metrics related to patient needs.
  • Serve as a resource for patients, providing ongoing care information and support.
  • Utilize available resources to address patient needs and refer to appropriate internal or external services.
  • Participate in interdisciplinary team meetings and maintain required licensure and certifications.
  • Adhere to professional standards and complete all necessary documentation.
