The Engagement Nurse Practitioner embodies the values and mission of WellBe in delivering care to our patients. This role adheres to the regulations set forth by state and federal agencies, ensuring compliance in all aspects of care delivery. As an Advanced Practice Provider, you will be integral to our interdisciplinary team, providing compassionate care to patients with both chronic and acute conditions while supporting their desire to remain in their homes.
Engagement Nurse Practitioners are responsible for onboarding eligible patients into the WellBe care model, conducting initial assessments, and establishing a comprehensive medical record for each patient. This market-level position requires travel to various patient locations throughout the day, emphasizing the importance of delivering timely and high-quality care.

- Conduct in-home engagement visits to assess and support patient care needs.
- Facilitate patient onboarding into the WellBe program, ensuring ongoing engagement and support.
- Collaborate with other healthcare professionals to manage the health and well-being of patients in their homes.
- Provide educational resources to staff, patients, and families to aid in decision-making.
- Perform initial screenings and physical examinations, develop appropriate care plans, and maintain timely documentation.
- Address urgent care needs and assist with transition of care visits as necessary.
- Coordinate with community teams to conduct initial patient assessments and care planning.
- Order and interpret diagnostic tests, determining follow-up needs and treatment options.
- Prescribe and manage medications in accordance with state regulations.
- Ensure accurate patient records and documentation at each appointment.
- Deliver high-quality care while adhering to company metrics related to patient needs.
- Serve as a resource for patients, providing ongoing care information and support.
- Utilize available resources to address patient needs and refer to appropriate internal or external services.
- Participate in interdisciplinary team meetings and maintain required licensure and certifications.
- Adhere to professional standards and complete all necessary documentation.
- Master's Degree in Nurse Practitioner studies from an accredited program.
- Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the relevant state.
- Current board certification (AANC or AANP).
- BLS certification and active NPI number.
- CPR certification.
- Active DEA licensure or eligibility to apply upon hire.
- Experience in home care, primary care, hospice, palliative care, or geriatrics is preferred.
- Valid driver's license and proof of auto insurance, with willingness to travel to patient locations.
- Ability to conduct in-home patient visits effectively.
- Proficiency in Microsoft Office and Electronic Health Records.
- Strong communication skills, both verbal and written.
- Experience in managing chronic and urgent medical conditions in older adults.
- Home care experience is advantageous.
Travel Requirements: This position may require local travel to deliver patient care.
Work Conditions: Ability to lift up to 30 lbs and assist with patient transfers as needed.
