Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality, patient-centered care in the comfort of patients' homes. As a member of our team, you will conduct comprehensive health assessments, develop personalized care plans, and collaborate with healthcare professionals to ensure seamless care coordination.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Masters degree required or commensurate experience and satisfactory completion of NP licensure.
- Masters degree required or commensurate experience and satisfactory completion of PA licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
- Ability to travel, valid state driver's license, and adequate automobile insurance coverage.
PRN/Part Time providers are non-benefited, except for 401(k) and limited voluntary benefits. Mileage reimbursement plan available.
