Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Care Technician 2 to join our team at Nuvance Health. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients and families.
Key Responsibilities- Assist with patient care activities and unit tasks under the supervision of licensed professional staff.
- Maintain all aspects of Nursing Assistant duties and demonstrate competency in additional functions such as monitor tech, video monitor tech, unit coordinator, and transfer mobility coach.
- Implement nursing orders promptly and accurately, ensuring a high level of patient care.
- Perform unit-related activities and clerical duties to support the nursing staff.
- Order and maintain necessary supplies and equipment for the unit, ensuring they are readily available and in working order.
- Maintain confidentiality of patient information and patient records.
- Coach new staff in service excellence standards and formulate effective working relationships with healthcare team members, patients, and significant others.
- Work as a Safety Attendant by providing a safe environment for specific patients.
- Appropriately use all communication technology and maintain a safe and orderly environment.
- Follow WCHN standards of service excellence and maintain 6 hours of continuing education annually.
- Fulfill all compliance responsibilities related to the position.
- High School Diploma and a minimum of one year of job-related experience.
- Current PEP rating at meets expectations or better.
- Maintain skills and competencies necessary for the position.
- Certified Nursing Assistant preferred.
- Maintain competency for Basic Life Support.
This role requires manual skills, motor coordination, and finger dexterity. There is some occupational risk, and the physical effort is very heavy, requiring exertion of up to 50 lbs. force. The physical environment involves significant exposure to dirt, odors, noise, human waste, etc.
