Job Summary

This role focuses on providing professional and non-professional nursing care services in accordance with physician orders.

In addition, this role focuses on performing the following Clinical Nurse duties:

  • Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
  • Provides professional nursing care to patients. Requires a nursing license.
  • A professional individual contributor role that may direct the work of other lower-level professionals or manage processes and programs.
  • The majority of time is spent overseeing the design, implementation, or delivery of processes, programs, and policies using specialized knowledge and skills typically acquired through advanced education.
Key Responsibilities
  • Managing projects/processes, working independently with limited supervision, coaching, and reviewing the work of lower-level professionals, resolving difficult and sometimes complex problems.
  • Provides leadership within nursing by providing direction and priority setting in regards to patient care.
  • Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research, and inpatient partners, thus producing high-performing, patient-focused teamwork.
  • Acts as a resource for all staff in regards to education, planning, relocating, and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population.
  • Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope.
  • In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.
  • Identifies high-risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
  • Promotes shared decision-making with patients in regards to healthcare; educates the patient and/or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions.
  • Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
  • Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.
  • Demonstrates compliance with regulatory requirements, i.e., MIPS, fall assessment.
  • Participates in satellite quality and safety initiatives as indicated.
