Advanced Practice Provider in Hospital Medicine

POSITION SUMMARY
The Advanced Practice Provider (APP) in Hospital Medicine delivers comprehensive care in alignment with the established guidelines, protocols, and regulations of LMH Health, ensuring the confidentiality and integrity of patient information.
This role is pivotal in orchestrating patient care by utilizing sound clinical judgment based on the healthcare needs and priorities of patients and their families.
KEY RESPONSIBILITIES
Foster relationships centered around patient and family involvement, aiding in decision-making through thorough assessments of medical histories, symptoms, evaluations, diagnoses, conditions, and preferences.
Engage in clear and continuous communication with the clinical team, including physicians, both verbally and in written documentation, addressing identified patient issues and needs by implementing and overseeing care plans, while accurately documenting and relaying patient conditions to enhance health outcomes.
Evaluate and coordinate discharge planning needs in collaboration with the healthcare team members.
Collaborative Teamwork and Leadership:
Actively participate as a member of an inter-professional team to deliver patient-centered care that achieves quality outcomes by:
- Leveraging information, resources, and technology
- Implementing Patient-Centered Care Nursing processes, including family involvement, Evidence-Based Practices (EBP), safety measures, and process enhancements
PROFESSIONALISM:
Demonstrate a commitment to lifelong learning; contribute to the advancement of nursing science; engage in governance; share knowledge to broaden the understanding of best practices; and uphold the ethical, moral, and regulatory foundations for exemplary patient and unit care.
Deliver care in accordance with the population served on the unit as outlined in the Administrative Scope of Services Policy.
Perform additional duties aligned with the current scope of practice for nurse practitioners.
QUALIFICATIONS
Preferred:
- A minimum of 2 years of experience as an APP, particularly in Internal Medicine and/or Hospitalist Medicine.
- Self-directed and proactive learner and provider.
Required:
- Master's degree or Doctorate in Nursing and Board Certification
- Board Certified Physician Assistant with a valid license in the state of Kansas
- Current Advanced Practice Registered Nurse license to practice in Kansas
- Current Basic Life Support and ACLS certification for Healthcare Providers
