POSITION SUMMARY
The Advanced Practice Provider (APP) in Hospital Medicine delivers comprehensive care in alignment with the established guidelines, protocols, and regulations of the healthcare institution, ensuring the confidentiality of patient information.
This role is pivotal in orchestrating patient care by utilizing sound decision-making skills tailored to the healthcare needs and priorities of patients and their families.
KEY RESPONSIBILITIES
Foster patient and family-centered relationships, aiding in decision-making through thorough assessments of medical histories, symptoms, examinations, diagnoses, and patient preferences.
Engage in clear and continuous communication with the clinical team, including physicians, both verbally and in documentation, concentrating on identified patient issues and needs by implementing and overseeing patient care plans, as well as documenting and conveying patient conditions appropriately to enhance health outcomes.
Evaluate and coordinate discharge planning needs in collaboration with healthcare team members.
Team Collaboration and Leadership:
Effectively employs teamwork and leadership skills as a member of an inter-professional team to ensure patient-centered care that achieves quality outcomes by:
- Leveraging information, resources, and technology.
- Implementing Patient-Centered Care Nursing processes, including patient/family engagement, Evidence-Based Practices (EBP), safety, and process improvement.
PROFESSIONALISM:
Demonstrates a commitment to lifelong learning; contributes to the advancement of nursing science; participates in governance; shares knowledge to enhance best practices; and upholds the ethical, moral, and regulatory foundations for optimal patient and unit care.
Delivers care in accordance with the population served on the unit as outlined in the Administrative Scope of Services Policy.
Performs additional duties aligned with the current basic nurse practitioner scope.
QUALIFICATIONS
Preferred:
- A minimum of 2 years of experience as an APP, particularly in Internal Medicine and/or Hospitalist Medicine.
- Self-directed and motivated learner and provider.
Required:
- Master's degree or Doctorate in Nursing and Board Certification.
- Board Certified Physician Assistant with a valid license in the state of Kansas.
- Current Advanced Practice Registered Nurse license to practice in Kansas.
- Current Basic Life Support and ACLS certification for Healthcare Providers.
