Job Summary

Mental Health Association - Western MA is seeking a skilled Case Manager to provide support and services to individuals experiencing chronic homelessness. The successful candidate will work collaboratively with a multidisciplinary team to develop and implement individualized action plans, ensuring participants achieve housing stability and independence.

Key Responsibilities
  • Intake and Assessment: Coordinate documentation for program entry, including chronic homeless verification, VI-SPDAT, and initial adult comprehensive assessments.
  • Service Planning: Develop and implement individualized action plans, working collaboratively with participants and the treatment team to establish and meet goals.
  • Housing Stability: Assist participants in securing safe housing, develop and maintain relationships with landlords, and provide tenancy support.
  • Income and Benefit Maximization: Assist participants in securing and maintaining employment, accessing benefits and resources, and managing finances.
  • Community Resources and Access: Promote participant engagement in community life and resources, including libraries, recovery groups, and social activities.
  • Participant Health and Wellbeing: Assist participants in coordinating medical, dental, and psychiatric appointments, and provide support for healthy habits and physical activity.
  • Teamwork and Communication: Regularly attend team meetings, participate in supervision, and maintain open communication with the treatment team and participants.
Requirements
  • Education and Experience: Bachelor's degree in human services or relevant field preferred; six months of applicable experience, with a focus on working with individuals with mental health and/or substance use challenges or housing instability.
  • Skills and Abilities: Valid driver's license, good oral and written communication skills, ability to work independently and as part of a team, and strong interpersonal skills.
  • Certifications: Must be able to pass and maintain certifications in medication administration (MAP), CPR, and first aid.
