At HOSPICE of SURRY COUNTY, we're seeking a dedicated Registered Nurse Case Manager to join our team. As a RN Case Manager, you'll play a vital role in providing expert nursing care and emotional support to patients and their families.

Key Responsibilities:
  • Conduct comprehensive assessments to understand and address patients' physical, emotional, and spiritual needs.
  • Develop tailored care plans in collaboration with patients and their loved ones, ensuring comfort and dignity throughout their hospice journey.
  • Coordinate services and resources to optimize patient care and support.
  • Educate patients and families about hospice services, disease progression, and end-of-life care options.
  • Administer medications, manage symptoms, and adhere to clinical protocols with precision and empathy.
  • Work closely with physicians, nurse practitioners, social workers, and other healthcare professionals to enhance patient outcomes and quality of life.
Requirements:
  • Registered Nurse (RN) license in good standing.
  • Excellent communication and problem-solving skills.
  • Ability to work independently and manage time effectively.
  • A compassionate and patient-focused approach to care.
