Job Title: Field Nurse Case Manager

We are seeking a skilled and compassionate Field Nurse Case Manager to join our team at Pivotal Placement Services. As a Field Nurse Case Manager, you will play a critical role in helping patients navigate our healthcare system more efficiently and effectively.

Key Responsibilities:
  • Meet with patients at physician offices during their recovery process to provide guidance and support.
  • Utilize your nursing expertise to assess patient needs and develop personalized care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Provide education and resources to patients and their families to promote healthy outcomes.
Requirements:
  • Registered Nurse (RN) license
  • Previous field case management experience
  • Ability to use Microsoft Office Suite effectively (Outlook, Word, Excel)
