We are seeking a skilled Case Manager to join our Time Limited Rental Assistance Program team at Good Shepherd Services. As a Case Manager, you will play a critical role in providing comprehensive support services to young adults experiencing homelessness, helping them to achieve stability and independence.
Key Responsibilities- Provide individualized case management services to program participants, including counseling, advocacy, and group work.
- Conduct biopsychosocial assessments to identify participants' strengths, needs, and challenges.
- Develop and implement individualized action plans and goals with participants.
- Identify and connect participants with community resources and services to support their housing stability and overall well-being.
- Collaborate with program staff and external organizations to ensure seamless service delivery.
- Document services and contacts in a timely and accurate manner.
- Bachelor's degree in a related field, such as social work or counseling.
- At least two years of experience providing services to at-risk youth, including those experiencing homelessness.
- Strong knowledge of community resources and services, particularly those related to housing and youth development.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
