Job Summary

We are seeking a skilled Care Manager Assistant to join our team at Maui Medical Group. As a key member of our staff, you will play a vital role in ensuring patients receive high-quality care that is both effective and cost-efficient.

About the Role

The ideal candidate will have 2-5 years of experience working in a clinical setting and hold a Medical Assistant Certificate. You will be responsible for managing multiple tasks simultaneously, maintaining a positive attitude, and working overtime as needed.

Key Responsibilities
  • 40% Facilitate collaboration between Primary Care Physicians (PCPs) and clinic staff to manage high-risk patients by providing timely information on preventive screenings and progress. Assist patients with referrals for Behavioral Health, Healthcare Management, Tobacco Cessation, and Substance Abuse counseling. Track compliance and follow-through with referrals.
  • 15% Participate in team meetings and quality improvement initiatives. Assist with patient data collection, health outcomes reporting, clinical audits, and programmatic evaluation.
  • 15% Utilize management and reporting tools to educate PCPs and clinic staff on best practices. Understand and apply utilization management reports to identify care gaps.
  • 10% Possess knowledge of Medicare's chronic care guidelines and Medicare's Innovation Programs and Delivery Guidelines, including billing associated with these guidelines.
  • 10% Perform miscellaneous responsibilities and duties as assigned.
  • 5% Demonstrate exceptional work quality, complete assignments on time, and make decisions within limits of authority. Take initiative in problem-solving, maintain confidentiality, and interact with patients, coworkers, and management in a courteous and respectful manner.
  • 5% Maintain consistent attendance and adhere to company policy.
Educational Requirements

Minimum qualifications include a Medical Assistant Certificate and 2-5 years of experience in a clinical setting. Desired qualifications include effective communication and telephone skills, proficiency in Microsoft Office and Coreo.
