About the Role

We are seeking a skilled Occupational Therapist to join our team at Revival Home Health & Hospice, a part of BrightSpring Health Services. As an Occupational Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Responsibilities
  • Assess and analyze patient care needs to develop comprehensive plans of care, including discharge planning.
  • Conduct thorough assessments of the patient's environment to ensure a safe and effective delivery of care.
  • Identify therapy diagnosis/patient care problems and interventions, making additions or changes as necessary to ensure comprehensive addressing and progress documentation.
  • Implement services and programs related to Occupational Therapy, ensuring orders are carried out in a professional manner satisfying professional ethics, patient rights, and state and federal codes and regulations.
  • Document additions and/or changes to the patient plan of care, re-evaluating progress and making changes as necessary under the physician's direction.
  • Provide patient and caregiver education relating to diagnosis, medications, infection control, and safety measures in accordance with the plan of care.
  • Conduct comprehensive medication reviews and ensure patients and their caregivers receive information regarding the patient's medication regimen.
  • Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.
  • Provide all aspects of case management to enhance patient outcomes.
  • Complete and submit all required documentation within established agency guidelines.
Requirements
  • Graduate of an Occupational Therapist Education program approved by ACOTE.
  • Current Occupational Therapist licensure in the State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire.
  • Two (2) years of experience in adult occupational therapy, home health experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and occupational therapy skills per competency checklist.
