Key Responsibilities

The RN Case Manager will be responsible for:

  • Conducting thorough initial assessments to determine patient needs and appropriate care levels.
  • Collaborating with multidisciplinary teams to ensure quality patient care throughout the hospital stay.
  • Developing and updating individualized care plans based on patient assessments and ongoing evaluations.
  • Coordinating discharge planning with both internal and external healthcare providers while maintaining patient confidentiality.
  • Documenting all care activities in compliance with quality standards and regulations.
  • Advocating for patients and their families in a respectful and confidential manner.
  • Serving as a resource for the healthcare team regarding case management practices.
  • Referring complex cases to appropriate management when necessary.
  • Ensuring compliance with Medicare requirements during the discharge process.
  • Addressing consults to case management in a timely manner.

Additional Responsibilities:

  • Working closely with social workers to identify and address social and financial barriers affecting patient care.
  • Participating in departmental meetings and contributing to various committees as needed.
  • Engaging in continuous professional development through educational opportunities and workshops.
  • Providing guidance to physicians and staff on financial and documentation matters.
  • Maintaining adherence to all organizational policies and HIPAA regulations.
  • Performing other related duties as assigned.
Qualifications

Required:

  • Current nursing license or eligibility for licensure in Kentucky.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Bachelor's degree in nursing or related field preferred; candidates with an Associate Degree in Nursing actively pursuing a BSN will be considered.
  • Minimum of 2-3 years of nursing experience, with a focus on utilization management or case management.
  • Strong organizational and communication skills, with the ability to manage multiple priorities effectively.

Preferred:

  • Certification in a nursing specialty or Case Management Certification (CCM).
