Primary Care Nurse

As a Staff Nurse at Kaiser Permanente, you will provide high-quality nursing care to a defined group of patients on an on-call basis. You will adhere to our ambulatory Nursing Care Standards, the Nursing Code of Ethics, and our policies.
Duties and Responsibilities:- Assessment and Planning: Using the nursing process, you will assess the nursing care needs of a defined group of patients. You will develop a written plan of care for patient care needs, prioritize patient care based on essential patient needs, and provide ongoing evaluation of patients' responses to care.
- Patient Education: You will teach and counsel patients and families/significant others in developing health care goals that are part of the individualized plan of care. You will also identify patients' educational needs as part of the nursing assessment.
- Care Coordination: You will work with other healthcare team members in planning and implementing care. You will coordinate and facilitate healthcare services for patients in other segments of the healthcare delivery system.
- Health Promotion and Preventive Aspects: You will assess patients' lifestyle risk factors, integrate health promotion and preventive aspects of care with ongoing assessment of nursing care needs, and promote patient self-care responsibility for meeting their own health needs.
- Licensure: Registered Nurse License (Washington) required at hire
- Education: Associates degree in Nursing from an accredited program
- Skills: Effective communication, problem-solving, leadership, and decision-making skills
- Bachelors degree in nursing
- Ambulatory care nursing experience
- Primary nursing or case-management experience
