Job Overview

CareOregon is seeking a Primary Care Nurse Specialist to join our team. This role plays a crucial part in promoting preventive care and addressing health concerns to enhance the quality of life for patients and their families.

Key Responsibilities
  • Provide direct patient care support in primary care settings, utilizing primary and palliative care principles, trauma-informed care, and promoting patient well-being and empowerment.
  • Coordinate annual wellness visits with patients, families, and caregivers, and perform medication reconciliation through patient interview, review of medical records, and consulting with other healthcare providers.
  • Collaborate with providers to resolve discrepancies or address care gaps and recommend preventative measures such as mammograms, colonoscopies, lab tests, etc.
  • Engage patients and family in advance care planning, including discussions about goals of care and answering questions about advanced directives.
  • Education patients about their health conditions, treatment options, and self-management strategies to promote active participation in their care.
  • Coordinate and accurately perform point-of-care testing, communicate findings to the healthcare team, and provide test results to patients when appropriate.
Essential Qualifications
  • Licensed Practical Nurse (LPN) with current licensure and certification.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Strong communication and interpersonal skills, with ability to work collaboratively with interdisciplinary teams.
  • Basic computer skills and proficiency in electronic health records (EHRs).
Salary and Benefits

The estimated salary range for this position is $68,000 - $83,000 per year, with a 5% bonus target and full benefits package.
