Job Summary

The Licensed Practical Nurse (LPN) at Sholom Community Alliance plays a vital role in ensuring the highest practical level of physical, mental, and psychosocial well-being for our residents. As a key member of our interdisciplinary team, you will provide direct care to residents, administer medications, and perform quality personal care.

Key Responsibilities

* Provide individualized care to residents, administering medications and treatments as prescribed by doctors.
* Act as an advocate for resident privacy and dignity, ensuring a comfortable and respectful care environment.
* Monitor resident vitals, completing assessments as assigned, and communicate changes to the RN Nurse Manager, LPN Clinical Coordinator, Clinical Lead, medical provider, and family members as instructed.
* Collaborate with the IDT to promote efficient service delivery and ensure a smooth transition of new residents.
* Perform focused assessments of residents with acute changes in condition, reporting changes to the medical provider.
* Attend resident care conferences as needed and provide guidance to NARs and TMAs.
* Communicate expectations and monitor unit personnel's assignment completion.

Requirements

* Must be licensed as a Licensed Practical Nurse in the State of Minnesota and maintain licensure.
* One or more years of experience in long-term care is highly preferred.
* Current certification in CPR is required.

Work Environment

The work environment is indoors, and the incumbent must be flexible about switching positions quickly to respond to resident needs and emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious materials.
