We are seeking a highly skilled and compassionate Ambulatory Nurse Specialist to join our team at Thomas Jefferson University Hospital. As a key member of our neurosurgery department, you will play a vital role in providing exceptional patient care and contributing to the success of our team.
Key Responsibilities- Provide high-quality patient care in a fast-paced ambulatory setting, utilizing evidence-based practices and clinical expertise to address the physical, psychological, and social needs of patients.
- Collaborate with neurosurgical providers to ensure seamless communication and coordination of patient care, supporting efficient and timely communication between primary care providers and specialists.
- Develop and implement clinical care pathways for common neurosurgical conditions, participating actively in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Provide patient education on preventive care, chronic disease management, and medication management, as well as education on common illnesses treated in the neurosurgical setting.
- Document patient interactions and clinical care in the electronic medical record, adhering to organizational standards and regulatory requirements.
- Perform care coordination for patients, including abnormal results, triage, RN verification of vaccinations, and management of in-baskets.
- Oversee clinical care essential to the assessment, promotion, maintenance, and restoration of patients' health and wellness.
- Minimize risk of harm to patients by following established nursing practices and protocols.
- Utilize information technology to communicate, manage knowledge, and mitigate errors.
- Use data to monitor outcomes and support ethical decision-making, using improvement methods to design and test changes to continuously improve patient care.
- Function effectively within inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
- Recognize patient preferences and cultural beliefs, providing safe, compassionate, and coordinated care.
- Create an environment of belonging and inclusion, demonstrating skills in problem-solving and conflict resolution.
- Demonstrate a commitment to learning and seek opportunities for continued professional growth and development.
- Participate in team huddles and assemblies to provide announcements and clinical operations updates.
- Be willing to give and receive feedback and integrate appropriate changes into practice.
- Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
- 2+ years' experience as a Nurse, in primary care or ambulatory setting preferred.
- Current RN licensure in state of employment; AHA BLS certification.
