Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes.

Key Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care
  • Collaborate with primary care physicians on patient education and provide follow-up care
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
Requirements
  • Master's degree in nursing or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Current BLS, ACLS, or CPR certification
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health/medical record software
  • Excellent verbal and written communication skills
Travel Requirements

Ability to travel to meet patients in their homes, valid state driver's license, and adequate automobile insurance coverage.
