About the Position:

As a Nurse Practitioner on our team, you'll conduct comprehensive health assessments, reviewing and observing members' current health, medical history, medication adherence, social environment, and other risks. This provides unparalleled insight into a member's overall health and well-being, helping us improve quality of care and close multiple care gaps with a single visit.

Responsibilities:

  • Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
  • Collaborate with Primary Care Physicians on patient education and provide follow-up.
  • Provide services in various venues, including Home Visits, Skilled Facility Visits, and Telehealth Visits.

Qualifications:

  • Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with Employee Health/Medical Record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent change.

Travel Requirements:

  • Able to travel, with a valid state driver's license and ability to drive a car, with proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.

Estimated Salary: $90.00 or $95.00 per completed Assessment, based on county, with opportunities for additional compensation and benefits.

Location: Home, SNF, Telehealth, and other community environments in Mansfield, Ohio, with flexible scheduling and opportunities for remote work.

Benefits: PRN/Part Time providers are non-benefited, except for 401(k) and limited Voluntary Benefits, with mileage reimbursement and opportunities for professional growth and development.
