We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Ochsner Home Health of New Orleans. As a LPN, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.

Responsibilities:
  • Provide high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Continually observe and report the patient's nursing needs to the supervising RN.
  • Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
  • Document patient visits thoroughly and completely per policy and payer requirements.
  • Actively participate in the performance improvement plan to reduce patient hospitalizations and improve outcomes.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
  • At least one year of experience working as an LPN.
