Job Summary: We're seeking a skilled Licensed Practical Nurse to join our team at ChenMed. As a key member of our care team, you will provide high-quality patient care and support our mission to transform healthcare for seniors.

Key Responsibilities:

  • Measure and record patient vital signs, interview, and medical history.
  • Catalog and communicate patient needs to the PCP, Clinician, Specialist, and other clinical personnel.
  • Prepare patients for examinations, perform routine screening tests, and assist physicians with exams.
  • Observe patients and report any changes in patient conditions to the PCP, Clinician, Specialist, and other clinical personnel.
  • Perform non-critical medical and therapeutic procedures based on medical instructions.
  • Maintain supplies, equipment, stocks, and sterilize instruments, practicing OSHA safety standards.
  • Process patient phone messages, returning calls and routing them to other team members as appropriate.
  • Provide health coaching to a defined group of patients to support healthy lifestyle choices.

Requirements:

  • High school diploma or equivalent required.
  • Graduation from a nationally accredited school for practical or vocational nursing required.
  • Current, active LPN license to practice in the state of employment required.
  • A minimum of 1 year of work experience as an LPN required.
  • A minimum of 3 years overall clinical experience is preferred.
