Job Summary:

The Registered Nurse Case Manager is a vital member of our team at Nascentia Health, providing direct patient care and coordination of overall home care services. This role requires a strong commitment to quality and safety, ensuring that all documentation meets the highest standards.

Key Responsibilities:

• Ensure quality and safe delivery of home health services, including case managing client caseloads and developing plans of care based on client assessment and OASIS evaluation.

• Complete all OASIS evaluations and documentation, ensuring that nursing care and interactions are accurately reflected and that documentation is clear, concise, and completed in a timely manner.

• Coordinate with LPNs, providing guidance and education on client care, and work collaboratively with the Clinical Supervisor and Patient Care Coordinator to efficiently manage patients using a team approach.

• Supervise paraprofessional staff, including home health aides, in performing designated tasks related to patient care and per the care plan, and communicate relevant information to the home health aide department.

Requirements:

• Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.

• Current and valid driver's license and auto insurance, and a vehicle which can be used for work (we do have a Fleet Program for field clinicians).

• Self-directed and organized with the ability to work independently in a fast-paced environment.

• Previous home care/case management experience a plus.

• Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
