Job Summary
The Home Health Licensed Practical Nurse will provide direct patient care in the home setting, working closely with our interdisciplinary team to develop and implement individualized care plans. This is a full-time position based out of our Nashville office.
Responsibilities
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.
- Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
- Current, valid, and unencumbered Tennessee LPN license or Compact license.
- Current and valid driver's license, transportation, and auto insurance.
- One or more years of clinical experience preferred.
- Self-motivated and customer service-oriented individual.
