Intake Specialist Job Description

Options for Community Living is seeking a skilled Intake Specialist to join our team. As an Intake Specialist, you will play a critical role in ensuring a smooth and efficient client admissions process.

Key Responsibilities:
  • Conduct thorough intake interviews and write comprehensive summaries.
  • Assist with all phases of pre-admission case management, including obtaining necessary documentation and arranging transportation.
  • Provide transportation for clients to visit sites and facilitate admissions.
  • Participate in data collection and reporting regarding referral applications, admissions, discharges, and transfers.
  • Communicate with service providers to update the status of pending applications.
  • Participate in client discharge meetings and intra-agency transfer meetings.
  • Conduct biannual transitional group meetings.
  • Support supervisors in admission documentation and coordination of benefits.
  • Maintain inactive file records and assist with internal admission meeting forms.
  • Complete assigned discharge follow-ups and assist with maintaining the criminal sex offender background binder and HUD books.
  • Participate in outreach events and presentations to increase referral activity.
Requirements:
  • Meet standards for designation as Qualified Mental Health Staff (QMHS).
  • Minimum of 2 years of college in a human services related discipline and 2 years of experience working in a setting with a disabled population.
  • Good writing skills and assessment abilities.
  • Valid driver's license, safe driving record, access to a vehicle, and valid auto insurance.

Salary and Schedule:

Salary range: $43,680/yr. ($24.00/hr.) - $54,600/yr. ($30.00/hr.). Salary offers will be commensurate with experience and other qualifications. Schedule: Monday - Friday, 8:00 AM - 4:00 PM, 8:30 AM - 4:30 PM, or 9:00 AM - 5:00 PM (35 hours/week). Location: In-person based out of our Ronkonkoma office, with field visits required in Nassau and Suffolk County. Pay Type: Non-exempt.
