($30.00/hr.)Salary offers will be commensurate with experience and other qualifications.Schedule:Monday - Friday: 8:00 AM - 4:00 PM, 8:30 AM - 4:30 PM or 9:00 AM - 5:00 PM (35 hours/week)Location: In-person based out of our Ronkonkoma office, with field visits required in Nassau and Suffolk County.Pay Type: Non-exemptResponsibilities:The Intake Specialist provides direct assistance to the Intake Department regarding client admissions, transfers and discharges from the agency. Serves as a support for supervisors for new admissions to ensure that all aspects of the admission plan have been completed. Conduct intake interviews and write intake summaries. Assists in all phases of pre-admission case management including obtaining all necessary documentation, arranging for day program transportation, and obtaining benefits. Provide transportation for clients to visit sites as well as actual admissions. Participate in the collection and reporting of data regarding referral applications, admission, discharges and transfers within the program. Contact Service Providers to update the status of pending applications. Participate in client discharge meetings and intra-agency transfer meetings. Conduct biannual transitional group meetings. Provide assistance with client admissions that present with difficult Medicaid and benefit issues. Support supervisors in all necessary admission documentation, physical exam, PPD, physician's authorization, application for benefits, coordination of benefits and securing client checks, completing admission forms and admission notes, obtaining medication and food and ensuring transportation of personal belongings if necessary. Coordinate with the Department of Social services and program staff to ensure inspections and security vouchers are obtained in a timely manner. Maintain inactive file records in accordance with the retention policy. Assist with the internal admission meeting form Complete assigned discharge follow ups as scheduled. Assist with maintaining the criminal sex offender background binder and HUD books. Participate in outreach events or presentations to increase referral activity. Assist with start-up of any new housing program. Any other appropriate tasks as assigned by management.
