Nurse Practitioner

Advantmed is seeking a highly skilled and compassionate Nurse Practitioner to join our provider network. As a Risk Adjustment Specialist, you will conduct in-home wellness risk adjustment assessments for Medicare and other populations, ensuring accurate and comprehensive risk adjustment coding and leading to greater value-based care.
Key Responsibilities- Perform annual wellness visits and health assessments on populations with chronic conditions
- Deliver patient health education opportunities
- Assist in closing quality care gaps (i.e. screenings and labs)
- Collaborate with the team to enhance care delivery
- Commit to a minimum of 30 hours per month
- Valid unencumbered NP License for the state of practice
- Previous in-home Risk Assessment experience preferred
- 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
- Bilingual is a plus
