Nurse Practitioner

Advantmed is seeking a skilled and compassionate Nurse Practitioner to join our provider network. As a Risk Adjustment Specialist, you will conduct in-home wellness assessments for Medicare and other populations, ensuring accurate and comprehensive risk adjustment coding.
This role is ideal for a motivated and detail-oriented individual who is passionate about delivering high-quality patient care. You will work closely with our team to assess the overall health and well-being of member beneficiaries, identifying opportunities for improvement and providing education on healthy lifestyle choices.
Key Responsibilities- Conduct annual wellness visits and health assessments on a population with chronic conditions
- Deliver patient health education opportunities to promote healthy behaviors
- Assist in closing quality care gaps, such as screenings and labs
- Collaborate with our team to enhance care delivery and improve patient outcomes
- Valid unencumbered NP License for the state of practice
- Previous in-home Risk Assessment experience preferred
- 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
- Bilingual is a plus
