Job Summary:

Kintegra Health is seeking a skilled Maternal-Child Case Manager to join our team. The successful candidate will be responsible for coordinating care and services for patients, utilizing a team approach to develop individualized goals that lead toward enhanced self-management.

Key Responsibilities:

  • Proficiently use data from Virtual Health to target patients and providers for outreach, education, and intervention.
  • Assist and advocate for patients to access care, services, and treatments in the most cost-effective healthcare setting.
  • Address the total patient and support systems, inclusive of medical, psychosocial, behavioral, cultural, and spiritual needs.
  • Involve the patient and their support systems in the decision-making process.
  • Utilize motivational interviewing techniques to measure a patient's understanding and acceptance of proposed plans.
  • Communicate and coordinate with all providers and members of the care team to minimize fragmented care.
  • Document clearly, completely, and concisely in Virtual Health related to all case management activities.
  • Provide coaching, information, and support to empower patients to make ongoing independent and/or healthy lifestyle choices.
  • Monitor quality and effectiveness of interventions for the patient by setting specific, measurable goals.

Requirements:

  • Associate degree or better from an accredited School of Nursing with an unrestricted license to practice nursing in NC.
  • Minimum of 5 years of nursing experience in an ambulatory or acute care setting, home health, or public health.
  • Case Management experience and CCM certification preferred.
  • Maternity nursing experience > 1 year strongly preferred.

Work Environment and Physical Demands:

  • Prolonged periods of sitting, reading, and documentation are required.
  • Intact sight and hearing with or without assistive devices if required.
  • Frequent walking for short distances.
  • Repetitive motion work with keyboards; limited physical activity.
  • Personal vehicle travel to patient home, provider office, offsite meetings.
