Job Summary:

We are seeking a highly skilled Inpatient Pharmacy Specialist to join our team at Kaiser Permanente. As a key member of our pharmacy team, you will be responsible for providing exceptional patient care and medication management services in a fast-paced hospital setting.

Key Responsibilities:
  • Monitor and manage medication therapy for hospitalized patients, including adjusting doses, ordering laboratory tests, and administering medications as needed.
  • Collaborate with medical personnel to develop and implement patient-specific plans to optimize medication therapy.
  • Conduct thorough assessments of patient medication regimens, identifying potential safety issues and implementing corrective actions.
  • Provide accurate and timely medication information to patients, families, and healthcare teams.
  • Participate in quality improvement initiatives to enhance patient safety and medication management practices.
Requirements:
  • Pharmacist License (California) required at hire
  • Advanced Cardiac Life Support certification within 3 months of hire
  • Basic Life Support certification within 3 months of hire
  • Controlled Substance Utilization Review and Evaluation System Registration required at hire
  • National Provider Identifier required at hire
Preferred Qualifications:
  • Doctorate of Pharmacy degree
