Job Summary

We are seeking a skilled Physical Therapist to join our team in providing community-based outpatient services to patients in the Peoria area. As a Physical Therapist with our organization, you will have the opportunity to work with a well-balanced mix of patients in the homes of older adults, providing one-on-one direct patient care and contributing to a highly experienced multidisciplinary therapy team.

Key Responsibilities
  • Provide community-based outpatient services to patients in the Peoria area
  • Work with a well-balanced mix of patients in the homes of older adults
  • Contribute to a highly experienced multidisciplinary therapy team
  • Perform ongoing 1-on-1 direct patient care
  • Regular and consistent weekly availability is required, which includes 3 weekdays of availability
Requirements
  • Graduation as a Doctor of Physical Therapy (or equivalent standing)
  • Eligibility for Illinois state licensure as a Physical Therapist
  • Must be available to work 12 hours per week
  • Some experience is required
