About the Role

VNA Health Group is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes.

Responsibilities
  • Assess and Evaluate Patients: Conduct thorough assessments and evaluations of patients to identify their physical therapy needs and develop personalized plans of action.
  • Develop and Implement Care Plans: Collaborate with physicians to develop and implement effective care plans that meet the unique needs of each patient.
  • Provide Skilled Therapy Services: Deliver high-quality physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote optimal health and well-being.
  • Monitor Progress and Outcomes: Regularly monitor patients' progress and outcomes, making adjustments to care plans as needed to ensure optimal results.
  • Communicate Effectively: Maintain open and effective communication with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
Requirements
  • Physical Therapy Licensure: Current licensure as a Physical Therapist in the state of New Jersey.
  • Experience: 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Valid Driver's License: Valid driver's license and reliable transportation.
