Position Overview

The Infusion Care RN Coordinator is a licensed professional nurse who serves as a clinical authority, delivering ethical, evidence-based, safe, and compassionate nursing care. This role emphasizes patient safety and the enhancement of nursing quality by assessing and clarifying patient requirements, executing clinic-related care, providing health education, advocating for patients, coordinating healthcare services, and evaluating patient outcomes.

Key Responsibilities

  • Oversee patient flow management, triage, and direct care.
  • Act as a communication liaison among front desk personnel, nursing staff, physicians, and clinic management.
  • Manage patient care by establishing expected outcomes, defining resource utilization, and collaborating with providers to set patient objectives.
  • Deliver nursing support and coordination to enhance patient outcomes and improve quality of life through clinical and educational initiatives.
  • Facilitate patient self-management of chronic conditions.
  • Ensure continuity of care for patients and families following hospital admissions, discharges, and emergency visits.
  • Handle high-risk patient care, including those with multiple co-morbidities or at risk for hospital readmission.
  • Build relationships with patients and utilize assessment skills to identify their physical, psychosocial, financial, and environmental healthcare needs.
  • Educate patients and relevant ancillary staff about healthcare needs, safety considerations, and benefits, including patient education.
  • Utilize electronic tools, such as EMR, for efficient documentation of assessments, clinical findings, treatment plans, and progress toward goals.
  • Monitor, document, and report on patients' conditions and responses to treatments and medications.
  • Maintain and review patient records, charts, and other relevant information, ensuring follow-up on tests and examination results.
  • Respond to patient inquiries related to treatment and provide instructions regarding medications and care.
  • Uphold patient confidentiality at all times.
  • Document patient care using EMR to maintain compliance with practice standards.
  • Demonstrate the professional, clinical, and technical competencies required for the assigned unit or department.

Required Qualifications

  • High School Diploma
  • Bachelor’s Degree in Nursing
  • Minimum of 2 years of RN experience

Preferred Qualifications

  • Previous experience in a medical office setting

Required Licensure and Certification

  • Basic Life Support Heartsaver (BLS HS) - American Heart Association (AHA) or obtained within 14 days
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing

Preferred Licensure and Certification

  • Active multi-state Registered Nurse (RN) license in good standing or active single state RN license with proof of application for a multi-state RN license.

Knowledge and Skills

  • Proficient typing and word processing skills
  • Strong analytical, critical thinking, and problem-solving abilities
