Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at A New Well, Inc. As an LPN, you will play a vital role in providing high-quality patient care and support to our clients in their homes.
Responsibilities- Provide personalized care to clients, including administering medications, performing vital signs, and assisting with daily living activities.
- Conduct thorough assessments of clients' needs and develop individualized care plans to ensure optimal outcomes.
- Collaborate with our interdisciplinary team to coordinate care and services, including home health aides, therapists, and other healthcare professionals.
- Communicate effectively with clients, families, and healthcare providers to ensure seamless care transitions and address any concerns or questions.
- Maintain accurate and detailed records of client care, including progress notes, medication administration records, and other relevant documentation.
- Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
- Current State of Georgia License as an LPN
- A minimum of one (1) year nursing experience preferred
- Home Health experience a plus
- Current CPR/First Aide certification
- Valid driver's license/State ID
- Negative PPB TB skin test within the last twelve (12) months
- Pass criminal background check
- Frequent lifting and repositioning of clients
- Repeated or prolonged standing, bending, kneeling, twisting, and occasional climbing of stairs
- Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell, and taste
- Agility and strength sufficient to handle clients and equipment without assistance
- Ability to respond physically and psychologically to emergency situations in the home or during transport
