We are seeking a highly motivated and compassionate Case Manager to join our Housing Navigation Services team at PATH (People Assisting the Homeless). As a Case Manager, you will play a critical role in assisting clients in breaking the cycle of homelessness by providing individualized support and connecting them to essential resources.
Key Responsibilities- Conduct Intakes and Needs Assessments: Meet with clients to understand their unique needs and develop personalized service plans to address housing goals, income, and other essential areas.
- Provide Referrals and Linkages: Connect clients with local resources, services, and support systems to help them achieve their goals.
- Evaluate Client Progress: Regularly assess client progress, identify areas for improvement, and adjust service plans as needed.
- Maintain Accurate Records: Ensure timely and accurate documentation of client interactions, service plans, and progress.
- Education and Experience: A minimum of 2 years of experience in healthcare, case management, homeless services, or a related field.
- Analytical and Communication Skills: Strong analytical and communication skills, with the ability to work effectively with diverse populations.
- Driving Requirements: Reliable transportation, a valid driver's license, and proof of insurance are required.
