Job Summary & Responsibilities

Nightingale's List is seeking a skilled and dedicated Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility in the San Jose area. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our facility.

Key Responsibilities:

  • Organize and provide necessary healthcare services consistent with the facility's policies and procedures.
  • Conduct basic hygiene and nursing care, including administration of prescribed medications and therapeutic treatments, as ordered by the physician.
  • Collect and report patient status information to the Primary Care Provider (PCP) or RN in a timely manner.
  • Provide patient education and accurately document care in the approved format, noting and carrying out physician's orders.
  • Administer and assist with therapeutic measures and treatments.
  • Clean and maintain equipment.
  • Obtain specimens for diagnostic testing as ordered.
  • Assist the RN and PCP with clinic appointments, including physical exams, minor surgeries, casting, and identifying minor illnesses.
  • Contribute to the treatment planning process and formal evaluation of patient progress by monitoring and recording observed patient behavior.
  • Participate in the training of patients in activities of daily living and assist in routine treatments.
  • Document nursing care and treatment provided and patient response in the patient health record to facilitate continuity of care and communication with other healthcare providers.

Requirements:

  • Twelve (12) continuous months of experience within the last three (3) years rendering direct patient care in a public or private institution. Clinical competencies must be current.
  • Possess a current and valid license issued by the California State Board of Vocational Nurse Examiners.
  • Possess current Basic Life Support (BLS) certification from an American Heart Association Provider or American Red Cross (with AED).
