Job Summary

We are seeking a skilled Care Coordinator RN to join our team at Westchester Medical Center. As a key member of our healthcare team, you will be responsible for coordinating care and services for selected patients across the healthcare continuum.

Your primary focus will be on promoting effective utilization of resources and ensuring the highest quality of care for optimal clinical and resource outcomes. You will assess and implement effective discharge planning, identifying appropriate services and facilities across the continuum of care.

As a Patient Advocate, you will assume a leadership and communication role with patients, families, interdisciplinary team members, and providers of appropriate services. You will provide ongoing monitoring and evaluation of the plan across the continuum as appropriate.

Key Responsibilities:

  • Providing interpretation and support for patients awaiting placement for nursing home or physical rehabilitation
  • Appropriate counseling and referrals for treatment of alcohol abuse, drug abuse, child abuse, domestic violence, and other mental health issues
  • Serving as Patient Advocate and Hospice Liaison
  • Discharge planning, implementation, and liaison between hospital and community agencies and institutions
  • Assessment of patients taking into account age, specific service needs, and stages of growth and development
  • Locating community and local resources available to patients in the hospital and after discharge
  • Developing and implementing appropriate and well-coordinated referral procedures
  • Following through on referrals to ensure necessary treatment plans have been implemented

Requirements:

  • RN, graduate of an accredited school of Nursing or CSW preferred; or B.A. or B.S. in related field
  • Two (2) years hospital-based experience preferred; one (1) year QA/UR or Case Management experience preferred
  • Current knowledge of Federal and State regulations
  • PRI certification preferred
