Care Coordinator RN

At Westchester Medical Center, we are seeking a skilled Care Coordinator RN to join our team. As a key member of our healthcare team, you will be responsible for coordinating care and services for selected patients across the healthcare continuum. Your primary goal will be to promote effective utilization of resources and provide the highest quality of care for optimal clinical and resource outcomes.
Key Responsibilities
- Assess and implement effective discharge planning through identification of appropriate services and facilities across the continuum of care.
- Provide ongoing monitoring and evaluation of the plan across the continuum as needed.
- Collaborate with the interdisciplinary team to ensure seamless care transitions.
- Develop and implement appropriate and well-coordinated referral procedures with community resources.
- Follow through on referrals to ensure necessary treatment plans have been implemented.
Requirements
- RN, graduate of an accredited school of Nursing or CSW preferred.
- Two (2) years hospital-based experience preferred.
- Current knowledge of Federal and State regulations.
