Clinical Social Worker

Kaiser Permanente is seeking a skilled Clinical Social Worker - Case Manager to join our team at Largo Medical Center. As a key member of our health care team, you will collaborate with patients, families, and healthcare professionals to provide comprehensive care and support.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patient needs and develop personalized care plans.
- Collaborate with healthcare teams to coordinate care, ensure timely interventions, and promote optimal patient outcomes.
- Provide education and support to patients, families, and caregivers on health-related issues, including end-of-life care and advanced directives.
- Facilitate access to community resources, financial counseling, and other support services to enhance patient well-being.
- Monitor and analyze patient outcomes to identify areas for improvement and inform program development.
- Master's degree in Social Work (MSW) required.
- Minimum 3 years of clinical experience and 2 years in case management.
- Licensed Clinical Social Worker (LCSW) certification in Maryland, District of Columbia, and Virginia within 6 months of hire.
- Case Manager Certificate within 36 months of hire.
- Experience with computer software programs in a Windows environment.
- Knowledge of community systems and resources in the Mid-Atlantic area.
