About the Job

We are seeking a motivated and compassionate Case Manager II to join our team. As a Case Manager II, you will work closely with participants to develop and implement Individualized Service Plans, provide advocacy and support, and connect them with essential resources to achieve their goals.

Responsibilities:

  • Build relationships with participants, understanding their unique needs and circumstances.
  • Develop and implement comprehensive Individualized Service Plans, ensuring timely progress toward goals.
  • Provide expert guidance on accessing community resources, including benefits support, housing assistance, and healthcare services.
  • Collaborate with Housing Specialists to ensure seamless transitions into permanent housing.
  • Engage in outreach efforts to encourage program participation and promote housing stability.
  • Document all interactions and interventions, maintaining accurate and up-to-date participant records.
  • Stay informed about local resources, best practices, and policy changes affecting participants.

Requirements:

  • Bachelor's degree or equivalent experience in social services, psychology, or a related field.
  • 1+ years of case management experience, preferably with at-risk populations.
  • Excellent verbal and written communication skills, with ability to build rapport with diverse groups.
  • Strong problem-solving and conflict resolution skills, with a focus on de-escalation techniques.
  • Able to work independently and collaboratively as part of a dynamic team.
  • Proficient in Microsoft Office Suite and Google applications.
